Enrolling in Year K-6

Complete the online application form below, or you can pick one up from our Office, and email together with supporting documents to Boyare.PS@education.wa.edu.au

Each enrolment for this year must complete:

• Application for enrolment
• Enrolment form
• Consent form
• Student Health Care Summary
• Third party 2025 (Returning Students)
• Third party 2025 (New Students)

All enrolments must be accompanied by a copy of birth certificate and Australian Immunisation Register (AIR) Immunisation History Statement (not more than two months old).
